Predominately the consultancy service is to assist patients with a wound failing to heal. This service is based in the Hunter and will be offered state-wide. Our advanced wound management service can be provided in the patient’s home, doctor’s surgery or residential aged care facility in collaboration with the health care team.
Our experienced and skilled Clinicians will work with you and in collaboration with the health care team to:
- Evaluate and review your clinical requirements regarding wound care
- Develop a wound treatment plan with you using best evidence-based practice to achieve your wound care goals
- Provide specialist advice to other nursing staff and your care team who are involved in your day to day wound care
- Monitor and evaluate your treatment plan via Face-to-face consults and telehealth to ensure your wound care plan is working for your wound
